1. Contextualization – Situations Assessment – problems (occupational skills, care management strategies, affective and relational dimensions)
2. Planning – Preparing and programming the intervention according to the actors
3. Objective – Definition of intervention objectives that safeguard the expectations, obligations and aspirations of the institution, the users and the self-evaluation.
4. Implementation – Implementation of planned actions
5. Identification of the problem – Problems clarification and areas of
intervention
6. Evaluation – Evaluation of the results and of the supervision process
